Are you accepting new patients?


What insurance do you accept?

We accept most major insurance companies and it varies for each provider. Please refer to the provider's link on the right side of this page for specific details.

What if my insurance is not listed?

If your insurance is not listed and you have Out Of Network (OON) benefits, we will courtesy file claims to your insurance. You will be financially responsible for whatever your insurance does not pay. We are willing to make payment arrangements.

What do I need to bring to my initial appointment?

Please print out the necessary forms from our website. All forms are located on the home page. If you are unable to print the forms, please call our office as soon as possible so that we can mail the forms to you or plan to arrive early to complete the forms in our office.

How early do I need to arrive?

If you are unable to print and complete the forms from our website, please arrive at least 30 minutes before your scheduled session time. If all paperwork is complete we ask that you arrive 15 minutes prior to the start of your session.

Can I bring my child/ren to my appointment?

Children under the age of 12 may not be left in the waiting room unattended. You will need to bring another adult to sit with them or re-schedule your appointment.

Would I be able to speak with the provider alone, without my child being present?

Yes, but with the understanding that children under the age of 12 may not be left in the waiting room unattended.
